To schedule an appointment, please call (480-380-5116) or send us an e-mail with your name, contact phone number, and the three best times for your appointment. You may also inquire about whether we provide for your insurance. We will call you promptly to finalize your appointment.

You may also print and fill out the Health History and Insurance Information forms in advance and bring them with you to your exam.

Our privacy policy is also available online for you to review at your leisure in accordance with HIPPA regulations.
